It was just recently that a new syndrome, osteoporosis of advanced age, was recognized as a new entity with its own pathophysiology and potential treatments. Unfortunately, and despite the fact that hip fractures are mostly a consequence of this process, and apart from the first edition of this book, there are few publications about this subject. 
The first edition of this book demonstrated that there is a need to understand the particular features of osteoporosis in older persons, which is the main aim of this book. The institutions where the authors are affiliated are highly recognized as the main centers in aging bone research. As an innovation, the second edition includes some aspects of muscle and bone interaction focusing of the impact of age-related muscle loss (sarcopenia) on the pathogenesis of falls and fractures in older persons. Описание: The term Vitamin D designates a group of closely related seco-steroids with antirachitic activity. The two most important members of these are ergocalciferol (vitamin D2) and cholecalciferol (vitamin D3). About 80 per cent of the vitamin D supply in man is produced in the skin and only 20 per cent is provided by nutrition.

The first analogue of Vitamin D, dihydrotachysterol, was developed in the 1930s and used in the treatment of primary or secondary hypoparathyroidism. It then took 40 years to learn that Vitamin D was not the biologically active principle for healing bone disease. It has to be hydroxylated in the liver at position 25 to build 25-hydroxycholecalciferol and then finally in the kidney at position 1a to become 1a, 25-dihydroxycholecalciferol [= 1a, 25-(OH)2-D3].

Further generations of Vitamin D analogues have since been developed. The most important of these are the so-called D hormone 1a, 25-(OH)2-D3 (= calcitriol) and its analogue alfacalcidol (= 1a-OH-D3) which are highly effective drugs for the treatment of renal bone disease. Further successful indications for active Vitamin D analogues include hypoparathyroidism and osteomalacia.

With regards osteoporosis, the role of an insufficient Vitamin D supply or impairment in activation was underestimated for a long time. A breakthrough was achieved with studies showing that the prevalence of mild to moderate vitamin D deficiency was very high and increasing with age.

Описание: Ren Rizzoli Menopause is the time in a woman's life when reproductive capacity ends. Ovaries decrease their activity and the production of sex hormones ceases. This period may be associated with a large variety of symptoms affecting the cardiovascular and urogenital systems, as well as skin, hair and bone. Bone capital is accumulated by the end of the second decade and remains more or less constant up to the time of menopause. Sex hormone deficiency leads to accelerated bone turnover, a negative balance and microarchitectural deterioration, which compromises bone strength, thereby increasing bone fragility and, thus, fracture risk. By the age of 80, it is estimated that 50% of trab- ular bone will have been lost. Natural menopause occurs between the ages of 45 and 54 years all over the world. This age does not appear to have changed significantly over the centuries. In contrast, since the middle of the 19th century, life expectancy, particularly in women, has increased considerably, with most women living to the age of 80 years or more in many regions of the world. This means that at the age of 50 years, a woman will live for more than 30 years without bone protection by sex hormones. This r- resents more than one-third of a woman's life. At the age of 50 years, the lifetime risk to experience a fracture is about 50% (ie, one out of two women will have a fracture during this period).
