Описание: A distinguished panel of pediatric and infectious disease specialists present in an easily accessible format all the information needed to diagnose a wide variety of infections in both the pregnant woman and the neonate. Organized by specific infection or related infections, the book provides pertinent information about the epidemiology of each infection in the pregnant woman and her neonate, as well as the risk of transmission to the fetus or neonate, the spectrum of clinical disease, and a recommended approach to the diagnosis of the infection in both the mother and neonate. The emphasis is on understanding what tests to order and how to interpret the results, with the approach to diagnosis in each chapter encompassing, not only the pregnant woman, but also her neonate. The book covers a wide range of microorganisms that are both common and uncommon causes of congenital and perinatal infections, including the herpes simplex virus, HIV, toxoplasmosis, syphilis, CMV, tuberculosis, malaria, dengue, lymphocytic choriomeningtis virus, and parvovirus.

Preterm infants grow poorly after birth and very commonly develop ex utero growth restriction (EUGR). However, the risks and benefits of catch-up growth in preterm infants must be weighed, and evidence addressing this warrants examination. Perinatal Growth and Nutrition explores the reasons for EUGR and the long-term effects on developmental outcome and on metabolic risks. It provides clear information on the risks and benefits of faster post-natal growth and catch-up growth in preterm infants and offers tools for better assessment of growth and earlier identification of faltering growth.
This book is divided into three sections. The first section covers advances in preterm infant growth standards, diagnosis and causes of EUGR, and assessments of preterm infant diets. The second section considers the extensive human literature on the effects of in utero and ex utero growth restriction and catch-up growth on long-term metabolic outcomes--such as obesity, insulin resistance, type 2 diabetes, and cardiac disease--and long-term neurodevelopmental outcomes including cognition. It also examines evidence for the effect of growth on these outcomes in term and preterm infants. The final section of the book considers ways to reduce the incidence of EUGR in preterm infants and when EUGR does occur, to optimize catch-up growth. Topics include assessment of dietary requirements of the diverse population of preterm infants, examination of tools for prescribing nutrition to neonatal intensive care unit patients, consideration of whether to customize or generalize nutrient intake, and fortification of human milk. In addition, the last chapter proposes using a Z-score growth chart for improved interpretation of growth data.

Описание: Hemorrhage and thrombosis are major hazards for pregnant women and their newborn infants. This book is concerned with the developmental mechanisms, the diagnosis and treatment, as well as the prevention of these hemorrhagic and thrombotic disorders. The topics discussed in this volume, (1) perinatal hemorrhage in mothers and their offspring; (2) coagulation disorders complicating pregnancy; (3) neonatal intracranial hemorrhage; and, (4) vitamin K deficiency in the neonate, will help bridge the gap between basic scientists and clinicians and between the pediatri- cian and the obstetrician. Hopefully, all those concerned with preventing these disorders will be stimulated by the information and questions raised in the fol- lowing presentations. Описание: This book covers several aspects of perinatal tissue-derived stem cells, from theoretical concepts to clinical applications. Topics include functions and different sources, immunomodulatory properties, translational point of view, GMP facility design and manufacturing for clinical translation, therapeutic potentials, and finally ethical considerations. The text provides a brief review of each type of perinatal stem cells and then focuses on their multi- or pluripotent properties, regenerative capacity, and future therapeutic potential in regenerative medicine. Additionally, the book discusses GMP compliance in stem cell facilities and the manufacture of stem cells for clinical translation. The chapters are authored by world-renowned experts in the perinatal stem cell field. Perinatal Tissue-Derived Stem Cells: Alternative Sources of Fetal Stem Cells, part of Springer's Stem Cell Biology and Regenerative Medicine series, is essential reading for basic and clinical scientists, clinicians, and pharmaceutical experts working or conducting research in the fields of stem cell biology, molecular aspects of stem cell research, tissue engineering, regenerative medicine, and cellular therapy.

Описание: The state of health care isreflected by perinatal and neonatal morbidity and mortality as weIl as by the frequencies of long-term neurological and developmental disorders. Many factors, some without immediately recognizable significance to childbearing and many still unknown, undoubtedly contribute beneficially or adversely to the outcome of pregnancy. Knowledge concerning the impact of such factors on the fetus and survivinginfant iscritical. Confounding analysesofpregnancy outcome, especially these past 2 or 3 decades, are the effects of newly undertaken invasive or inactive therapeutic approaches coupled with the advent of high technology. Many innovations have been introduced without serious efforts to evaluate their impact prospectively and objectively. The consequences of therapeutic misadventures character ized the past; it seems they have been replaced to a degree by some of the complications of applied technology. Examples abound: after overuse of oxygen was recognized to cause retrolental fibroplasia, its restriction led to an increase in both neonatal death rates and neurologic damage in surviving infants. Administration of vitamin K to prevent neonatal hemorrhagic disease, particularly when given in what we now know as excessive dosage, occasionally resulted in kernicterus. Prophy lactic sulfonamide use had a similar end result. More recent is the observation of bronchopulmonary dysplasia as a complication of re spirator therapy for hyaline membrane disease. The decade of the eighties opened with the all-time highest rate of cesarean section in the United States.

Описание: Decision making in modem obstetrics and perinatology is con- trolled by a variety of different factors, but there is little doubt that in industrialized countries one of the most powerful single factors is the concern and fear of causing permanent brain damage in the unborn and newborn.
