The synthesized testosterone is exogenously administered compensated for by the steroid is not able to exogenously administered compensate for the deficit of steroid endogenously synthesized testosterone Figure 2 Intratesticular and serum testosterone concentrations after treatment with andro- gens or anabolic steroids in order to substitute for the peripheral androgen deficiency and to achieve azoospermia. Because this steroid is alkylated in position 17, toxic effects on liver function can not be excluded. Danazol offers no advantages as compared with other anabolic steroids; rather, disadvantages. Numerous experiments of this type have been performed during the last 40 or 50 years. The outcome in each case was more or less identical: with a certain dose of an androgen or anabolic steroid it is possible to inhibit spermatogenesis without interfering with other androgen-dependent func- tions, including libido (potentia coeundi) and accessory sexual glands. On the basis of this pharmacological-endocrinological background, androgens and anabolic steroids can be used for male fertility control, and several clinical trials have been performed during the last 10-15 years. Some of 1 these studies 2-23 are mentioned in Table 2.

Описание: Progesterone, the hormone "pro gestationem", plays a pivotal role in mammalian reproduction during almost all phases of the menstrual cyc1e and all stages of pregnancy. It is involved in the control of ovula- tion, prepares the endometrium for implantation, and, in later stages of pregnancy, is responsible for its maintenance by suppressing uterine contractility. The sudden withdrawal of progesterone action at the end of the nonfertile cycle leads to the constriction of spiral arteries and, in . turn, to menstruation in human beings and non human primates. The decrease in serum progesterone concentrations or its functional with- drawal in the myometrium and decidua are the most important events during parturition in various mammals. In the uterus, progesterone controls the growth and differentiation of endometrial and myometrial cells and regulates a variety of cell functions directly by either stimulating or inhibiting structural and functional proteins, but also indirectly by functionally opposing estra- diol action. In the nonpregnant uterus, there are different progesterone effects on uterine cell proliferation which vary among species. In the fertile cyc1e, progesterone regulates in synergism with estradiol the transport of the fertilized eggs and the c1eavage stage embryos through the oviduct and induces the secretory changes in the endometrium re- quired for implantation. During the period between ovulation and im- plantation, remarkable morphological and biochemical changes in the lumenal and glandular epithelial cells take pi ace under the influence of rising progesterone levels in the human and primate endometrium.

Описание: This seventh edition has been completely revised and updated, incorporating relevant WHO and national guidance documents: therefore imparting best evidence-based practice for all methods. Combined oral contraceptives (COCs) Based on the long-established evidence that 7 days of routinely not-taking pills in each cycle is too long for maintained ovarian suppression - and this necessarily leads to too little margin for errors in pill-taking - the authors recommend that providers switch to a new norm for all users of either: Tricycling, the 84/4 regimen, or totally continuous use (365/365), OR, for women who remain keen to see monthly pill-bleeds (which are completely unnecessary for health), one of the regimens (24/4 or 21/4) that shorten the contraception-non-taking time to 4 days New methods, and their importance or otherwise: Intrauterine system: Jaydess (R) Subcutaneous, self-injectable alternative to Depo-Provera: Sayana (R) Press 24/4 combined hormonal contraceptives: Zoely (R), Eloine (R) Diaphragm: Caya (R) Updates Quick starting and bridging (the Proving not Pregnant Protocol) Emergency contraception (EC), how advice differs for ulipristal acetate EC Drug metabolism (implications with norethisterone) and interactions (eg affecting lamotrigine) Question and answer format Important information boxes Unwanted side effects boxes Frequent patient questions at the end of relevant chapters Management advice Follow-up adviceComes with free e-book on ExpertConsult for the first time This seventh edition has been completely revised and updated, incorporating relevant WHO and national guidance documents: therefore imparting best evidence-based practice for all methods.
