Описание: As a consultant for healthcare associations working on all the healthcare laws from Medicare, the Health Manpower Act, Budget Reconciliation, Clinton healthcare efforts and the Affordable Healthcare Act, Donald Lavanty had a front-row seat from which to witness the politics and policies motivating each action. This volume was written from that experience, which was first documented in the course on the Political Aspects of Healthcare, employing the analogy that these various legislative actions to connect the system are like attempting to build a canal connecting the Amazon and Nile rivers: a long and difficult process. The title examines ongoing efforts to create and maintain a workable solution to the conflicts inherent in the US healthcare system from all angles, conflicts that arise due to the nature of our democratic government in which healthcare policies change based on the position of the voting majority in Congress at the time any action is taken.
Дополнительное описание: 1. Pre-Medicare.- 2. The Federal Government Enters the Healthcare Field.- 3. The Action Period of Healthcare Legislation.- 4. The Cost Realities and Political Events’ Impact on National Healthcare Action.- 5. The Era of Budget Politics.- 6. The Reagan Era

Описание: In the summer of 1964 medical professionals, mostly white and northern, organized the Medical Committee for Human Rights (MCHR) to provide care and support for civil rights activists organizing black voters in Mississippi. They left their lives and lucrative private practices to march beside and tend the wounds of demonstrators from Freedom Summer, the March on Selma, and the Chicago Democratic Convention of 1968. Galvanized and sometimes radicalized by their firsthand view of disenfranchised communities, the MCHR soon expanded its mission to encompass a range of causes from poverty to the war in Vietnam. They later took on the whole of the United States healthcare system. MCHR doctors soon realized fighting segregation would mean not just caring for white volunteers, but also exposing and correcting shocking inequalities in segregated health care. They pioneered community health plans and brought medical care to underserved or unserved areas.Though education was the most famous battleground for integration, the appalling injustice of segregated health care levelled equally devastating consequences. Award-winning historian John Dittmer, author of the classic civil rights history Local People: The Struggle for Civil Rights in Mississippi, has written an insightful and moving account of a group of idealists who put their careers in the service of the motto ""Health Care Is a Human Right.

Описание: The sudden call, the race to the hospital, the high-stakes operation-the drama of transplant surgery is well known. But what happens before and after the surgery? In Transplanting Care, Laura L. Heinemann examines the daily lives of midwestern organ transplant patients and those who care for them, from pretransplant preparations through to the long posttransplant recovery. Heinemann points out that as efforts to control healthcare costs gain urgency-and as new surgical techniques, drug therapies, and home medical equipment advance-most of the transplant process now takes place at home, among kin. Indeed, the transplant system effectively depends on unpaid care labor, typically provided by spouses, parents, siblings, and others. Drawing on scores of interviews with patients, relatives, and healthcare professionals, Heinemann follows a variety of patients and loved ones as they undertake this uncertain and strenuous “transplant journey.” She also shows how these home-based caregiving efforts take place within the larger economic and political context of a paucity of resources for patients and caregivers, who ultimately must surmount numerous obstacles. The author concludes that the many snags encountered by transplant patients and loved ones make a clear case for more comprehensive health and social policy that treats care as a necessarily shared public responsibility.   An illuminating look at the long transplant journey, Transplanting Care also offers broader insight into how we handle infirmity in America-and how we might do a better job of doing so.

Описание: Chronic Failures: Kidneys, Regimes of Care and the Mexican State is about Chronic Kidney Disease (CKD) and the relentless search for renal care lived out in the context of poverty, inequality and uneven welfare arrangements. Based on ethnographic research conducted in the state of Jalisco, this book documents the routes uninsured Mexican patients take in order to access resource intensive biotechnical treatments, that is, different modes of dialysis and organ transplantation. It argues that these routes are normalized, bureaucratically, socially and epidemiologically, and turned into a locus for exploitation and profit.  Without a coherent logic of healthcare access, negotiating regimes of renal care has catastrophic consequences for those with the least resources to expend in that effort. In carrying both the costs and the burden of care, the practices of patients without entitlement offer a critical vantage point on the interplay between the state, markets in healthcare and the sick body.
