HIV continues to be a profound challenge facing communities nationally and internationally. Until a vaccine or a cure is found, prevention remains a most crucial line of defense. However, the successes made to reduce exposure and transmission have not benefited all communities equally. HIV continues to affect vulnerable communities, and HIV-related health disparities are growing.
The work documented in Innovations in HIV Prevention Research and Practice through Community Engagement spotlights the effectiveness of community involvement to reduce HIV infections in the United States. This timely resource introduces the concepts of community engagement, partnership, and community-based participatory research (CBPR). Contributors provide detailed examples of these concepts in which diverse research partners blend their unique insights and skills to arrive at an authentic understanding of phenomena and inform the translation of best practices and processes to enhance equity in HIV prevention and treatment. Equitable interactive collaboration is central to these efforts, in which community members and representatives from organizations, the scientific and medical sectors, and other relevant agencies nurture long-term health improvement through sustained teamwork. Challenges and barriers to effective engagement are identified, as are characteristics of successful partnerships. Included in the book:
Details of a multigenerational HIV prevention intervention in a rural southeastern community.
The challenges and successes of developing, implementing, and evaluating an intervention for higher-risk predominately heterosexual black men in college.
The history of gay community involvement in HIV prevention and its contributions to the theory and current practice of engagement.
Next steps in the integration of HIV-related policy change and research.
Community engagement within American Indian communities.
Keys to sustaining a CBPR partnership to prevent HIV within ethnic, sexual, and gender minority communities.
Innovations in HIV Prevention Research and Practice through Community Engagement offers researchers and practitioners in public health, community health, and medicine guidance on community engagement that is both inspiring and realistic.
"Community engagement and knowledge continue to be essential to prevent HIV infections. This book is a compilation of the state-of-the-science of engagement and delves deeper into the meaning and utilization of community-based participatory research, with implications that reach beyond the HIV epidemic to public health and medicine in general."

Sudden cardiac arrest is the leading cause of death among adults, yet it need not be fatal. Though survival in most communities is very poor, a few communities achieve rates as high as 50%. Why are some communities so successful in snatching life from the jaws of death? Resuscitate! describes the steps any EMS system can take to improve cardiac arrest survival. It is written for the medical directors, administrative directors, fire chiefs, dispatch directors, and program supervisor who direct and run EMS systems all across the country, and for the EMTs, paramedics, and dispatchers who provide frontline care.
This second edition of Resuscitate! provides fifteen concrete steps to improve survival. Four steps will lead to rapid improvements at the local level and are relatively easy to implement. Six additional steps are more difficult to implement but also likely to improve survival. The remaining steps recommend changes at the national level.
Resuscitate! is the official textbook for the Resuscitation Academy, held twice a year in Seattle. Cosponsored by Seattle Medic One, King County EMS, and the Medic One Foundation, the Academy draws attendees from throughout the world for two intensive days of classes, demonstrations, and workshops to acquire the knowledge and tools to improve survival in their own communities. This new edition includes lessons learned from attendees of the Academy as well as from the faculty's evolving thoughts on how to measure performance and improve survival, one community at a time. It also includes an addendum on the Resuscitation Academy (resuscitationacademy.org).
For more than thirty years, Mickey S. Eisenberg M.D., Ph.D. , has played a leading role in developing King County, Washington's emergency response to cases of sudden cardiac arrest, a system recognized as among the very best in the nation. He is a professor of medicine at the University of Washington and serves as the medical director of King County Emergency Medical Services.

Описание:
This salient reference grounds readers in the theoretical basis and day-to-day practice of community-based health care programs, and their potential as a transformative force in public health. Centering around concepts of self-determination, empowerment, and inclusiveness, the book details the roles of physicians, research, and residents in the transition to self-directed initiatives and greater community control. Community-focused interventions and methods, starting with genuine dialogue between practitioners and residents, are discussed as keys to understanding local voice and worldview, and recognizing residents as active participants and not simply targets of service delivery. And coverage pays careful attention to training issues, including how clinicians can become involved in community-based care without neglecting individual patient needs.
Among the topics covered are:
Narrative medicine in the context of community-based practice.
Qualitative and participatory action research.
Health committees as a community-based strategy.
Dialogue, world entry, and community-based intervention.
Politics of knowledge in community-based work.
Training physicians with communities.
Dimensions of Community-Based Projects in Health Care challenges sociologists, social workers, and public health administrators to look beyond traditional biomedical concepts of care and naturalistic methods of research, and toward more democratic programs, planning, and policy. The partnerships described in these pages reflect a deep commitment to patients' lives, and to the future of public health.p>

Описание: As a low-income country, Ethiopia has made impressive progress in improving health outcomes. This report examines how Ethiopia's Health Extension Program (HEP) has contributed to the country's move toward Univeral Health Coverage (UHC), and to shed light on how other countries may learn from Ethiopia's experiences of HEP when designing their own path to UHC. HEP is one of the government's UHC strategies introduced in a context of limited resources and low coverage of essential health services. The key aspects of the program include the capacity building and mobilization of more than 30, 000 Health Extension Workers (HEWs) targeting more than 12 million model families, and the mobilization of "health development army" to support the community-based health system. Using the HEP-UHC conceptual model and data from Demographic and Health Surveys, the study examines how the HEP has contributed to the country's move toward UHC. During the period that the HEP has been implemented, the country has experienced significant improvements in many dimensions: in terms of socioeconomic, psychological, behavioral, and biological dimensions of the beneficiaries; and in terms of the coverage of health care services. The study finds an accelerated rate of improvements among the rural, less-educated, and the poor population, which is leading to an overall reduction in equity gaps and improvements in the equity indicators - including the concentration indices - that suggest a more equitable distribution of resources and health outcomes. The HEP in Ethiopia has demonstrated that an institutionalized community approach is effective in helping a country make progress toward UHC. The elements of success in the HEP include the emphasis on community mobilization which identifies community priorities, engages and empowers community members, and supports their ability to solve local problems. The other aspect of HEP is the emphasis on institutionalization of the activities, which addresses the sustainability of community programs through high level of political commitment, and effective coordination of national policies and leveraging of support from partners. These findings may offer useful lessons for other low income countries facing similar challenges in developing and implementing a sustainable UHC strategy.
