Описание: This text addresses the key issue of informal payments, or ‘red packets’, in the Chinese Healthcare system. It considers how transactions take place at the clinical level as well as their regulation. Analysing the practice from the perspectives of institutions and power structure, it examines how institutional changes in the pre-reform and reform era have changed the power structure between medical professions, patients and the Party-state, and how these changes have given rise and perpetuate the practice. Drawing from qualitative data from interviews of medical professionals, the author recognises the medical profession as a major player in the health care system and presents their perception of the practice as the taker of ‘red packets’ and their interactions with the patient and the state surrounding the illegal practice in an authoritarian power structure. The books considers the institutional reasons that motivate doctors to take, patients to give, and the government to 'tolerate' red packets, arguing that the bureaucratization of the medical profession, society of acquaintances and shortage of quality of medical services jointly create an institutional setting that has given rise to these informal payments. Contributing to a rounded understanding of the problems of healthcare reform in China, this book is a key read for all scholars interested in the issue of informal payments and healthcare politics in transition economies.

Описание: This book presents an advanced systematic mapping review (SMR) and state-of-the-art taxonomy of emergency departments (EDs). Focusing on the patients’ level of fulfilment and how it can be enhanced, it examines existing problems like waiting periods and overcrowding and how these can be alleviated to provide a better service. The author examined research papers from 1964 to 2018, and developed six research questions, organising them using mapping studies, the primary objectives of which were firstly, to obtain a common understanding of the problems that need to be highlighted in EDs, and secondly, to re-analyse the methods used.Focusing on quality, the book encourages citations of experimental methods from important studies concerning EDs that can improve services. Through different research papers, various thematic areas in the healthcare sector were examined, like the determination of the relative efficiency of pre-discharge interventions; the analysis of care and managing common indications during the last stages of life; using e-Health to enhance effectiveness and proficiency; the seriousness of patient differences among EDs; the identification of quality problems in healthcare contexts; existing opportunities and the suggested plans. The book concludes that an analytical decision-making process should be used to assess a health technology on the basis of its performance. It stresses the importance of updating this analytical system frequently.

Описание: This book focuses on justice and its demands in the way of providing people with medical care. Building on recent insights on the nature of moral perceptions and motivations from the neurosciences, it makes a case for the traditional medical ethic and examines its financial feasibility. The book starts out by giving an account of the concept of justice and tracing it back to the practices and tenets of Hippocrates and his followers, while taking into account findings from the neurosciences. Next, it considers whether the claim that it is just to limit medical care for everyone to some basic minimum is justifiable. The book then addresses finances and expenditures of the US health care system and shows that the growth of expenditures and the percentage of the gross national product spent on health care make for an unsustainable trajectory. In light of the question what should be changed, the book suggests that overdiagnosis and medicalizing normal behavior lead to harmful, costly and unnecessary interventions and are the result of unethical behavior on the part of the pharmaceutical industry and extensive ethical failures of the FDA. The book ends with suggestions about what can be done to put the U.S. health care system on the path to sustainability, better medical care, and compliance with the demands of justice.
