Описание: Based on extensive field research in the Manianga region of the Lower Congo, Health in a Fragile State is an anthropological account of public health and health care after the collapse of the Congolese state in the 1980s and 1990s. This work brings into focus John M. Janzen's earlier books on African health and healing, revealing the collaborative effort by local, national, and international agencies to create viable alternative institutions to those that represented the centralized state. This book documents and analyzes the realignment of existing institutions and the creation of new ones that shape health and healing. Janzen explores the manner in which power and information, including science, are legitimized in the preservation and improvement of health. Institutional validity and knowledge empower citizens and health practitioners to gain the upper hand over the region's principal diseases, including malaria, tuberculosis, typhoid, and HIV/AIDS.
Set in Tanga, a city on the Tanzanian Swahili coast, Dominik Mattes examines the implementation of antiretroviral HIV-treatment (ART) in the area, exploring the manifold infrastructural and social fragilities of treatment provision in public HIV clinics as well as patients' multi-layered struggles of coming to terms with ART in their everyday lives. Based on extensive ethnographic fieldwork, the book shows that, notwithstanding the massive rollout of ART, providing treatment and living a life with HIV in settings like Tanga continue to entail social, economic, and moral challenges and long-term uncertainties, which contradict the global rhetoric of the "normalization of HIV."
How do specific secular and religious ideologies—such as nationalism, neoliberalism, atheism, Pentecostalism, Tablighi Islam, and shamanism—gain popularity and when do they lose traction? To answer these questions, Mathijs Pelkmans critically examines the trajectories of a range of ideologies as they move into the post-Soviet frontier in Central Asia. Ethnographically rooted in the everyday life of a former mining town in southern Kyrgyzstan, Fragile Conviction shows how residents have dealt with the existential and epistemic crises that arose after the collapse of the Soviet Empire. Residents became enchanted by the truths of Muslim and Christian missionaries, embraced the teachings of neoliberal and nationalist ideologues, and were riveted by the visions of shamanic healers. But no matter how much enthusiasm and hope these ideas first engendered, the commitment to any of them rarely lasted very long.Pelkmans finds that there is an inverse relationship between the tenacity and the effervescence of collective ideas, between their strength to persist and their ability to trigger committed action. Introducing the concept of pulsation, he argues in Fragile Conviction that ideational power must be understood in relation to three aspects: the voicing of the idea, its tension with everyday reality, and its reverberation within groups of listeners. The conclusion that the power of conviction is rooted in the instability of sociocultural contexts is a message that has relevance far beyond urban Central Asia.
