Описание: People who inject drugs (PWID) are at higher risk for acquiring HIV infection than the general population in many countries. Injecting drug use is present in 148 countries including a wide array of low and middle income countries where HIV incidence is growing in recent years - even as it declines elsewhere. This study was undertaken as the third of a series of three economic analyses examining the global epidemics of HIV/AIDS in vulnerable populations. Informed by a critical review of epidemiological evidence and the most recent analyses of intervention efficacy, this work modelled the costs and impact of addressing the needs of PWID at scale in various epidemic contexts. These particular research questions were addressed:<ul><li>What are the modelled epidemic impact and costs of implementing a minimum adaptable package of evidence-based and human rights affirming services for prevention, treatment, and care of HIV/AIDS in PWID in globally representative epidemic scenarios?</li><li>What are the estimated returns on investment and costs of inaction of addressing these epidemics? </li></ul>Taken together, the findings indicate:<ul><li>Service coverage levels for ART and key harm reduction interventions with HIV prevention benefits are generally inadequate. Allocative decisions for HIV/AIDS should better reflect the burden of transmission and disease. </li><li>HIV transmission dynamics can be significantly reduced by scaling up a package of four key harm reduction interventions specific to PWID: Needle and Syringe Programs, Medically Assisted Therapy and HIV Counseling and Testing, as well as proportionate access in ART scale-up. Evidence-based and effective interventions for PWID should be funded and implemented - in the context of HIV prevention services for the general population, such as condom promotion. </li><li>Interventions for PWID - particularly in combination with treatment services - are cost effective or highly cost effective investment choices across the breadth of the global epidemic. The study presents compelling evidence for the most cost-effective approaches to minimising the transmission and impacts of HIV in this key population. This evidence may guide both operational design and policy dialogue in World Bank operations.

This book discusses globalization and its impact on human health. The population of the world grew from 1 billion in 1800 to 7 billion in 2012, and over the past 50 years the mean temperature has risen faster than ever before. Both factors continue to rise, as well as health inequalities. Our environment is changing rapidly, with tremendous consequences for our health. These changes produce complex and constantly varying interactions between the biosphere, economy, climate and human health, forcing us to approach future global health trends from a new perspective. Preventive actions to improve health, especially in low-income countries, are essential if our future is going to be a sustainable one. After a period of undeniable improvement in the health of the world's population, this improvement is likely to slow down and we will experience- at least locally - crises of the same magnitude as have been observed in financial markets since 2009. There is instability in health systems, which will worsen if preventive and buffering mechanisms do not take on a central role. We cannot exclude the possibility that the allied forces of poverty, social inequalities, climate change, industrial food and lack of governance will lead to a deterioration in the health of large sectors of the population. In low-income countries, while many of the traditional causes of death (infectious diseases) are still highly prevalent, other threats typical of affluent societies (obesity, diabetes, cardiovascular diseases) are increasing. Africa is not only affected by malaria, TB and HIV, but also by skyrocketing rates of cancer. The book argues that the current situation requires effective and coordinated multinational interventions guided by the principle of health as a common good. An entirely competition-driven economy cannot - by its very nature - address global challenges that require full international cooperation. A communal global leadership is called for.

Paolo Vineis is Chair of Environmental Epidemiology at Imperial College. His current research activities focus on examining biomarkers of disease risk as well as studying the effects of climate change on non-communicable diseases.

Описание: In 1895, after enduring two previous cholera epidemics and facing horrific hygienic conditions and the fear of another epidemic, officials in the Argentine province of Tucumán described their home as the “Poisoned Eden,” a play on its official title, “Garden of the Republic.” Cholera elicited fear and panic in the nineteenth century, and although the disease never had the demographic impact of tuberculosis, malaria, or influenza, cholera was a source of consternation that often illuminated dormant social problems.

In Poisoned Eden Carlos S. Dimas analyzes the social, political, and cultural effects of three epidemics, in 1868, 1886, and 1895, that shook the northwestern province of Tucumán to understand the role of public health in building the Argentine state in the late nineteenth century. Through a reading of medical and ethnographic material, Dimas shows that cholera became intertwined in all areas of the social fabric and that Tucumanos of all classes created public health services that expanded the state’s presence in the interior. In each outbreak, provincial powers contended with how to ensure the province’s autonomy while simultaneously meeting the needs of the state to eradicate cholera. Centering disease, Poisoned Eden demonstrates how public health and debates on cholera’s contagion became a central concern of the nineteenth-century Latin American state and promoted national cohesion.
