In 1991, Oregon embarked on a journey to improve health for all its citizens by radically re-thinking how to approach health care for long-term benefits. Over more than two decades, Oregonians have participated in a dialogue to create a new approach to solve the dilemma of providing high quality health care that is affordable and effective. Traditionally, health care reform looked at cutting people from care, cutting provider rates or cutting services. Oregon’s approach is unique in that it built a new system of delivery from the ground (community) up. . The Oregon model took a “Fourth Path” to health care by redesigning the clinical delivery system through reducing waste, improving individual health and prevention, and therefore reducing utilization of services, creating local accountability, aligning financial incentives and creating fiscal accountability. This is not only an Oregon story, but a national one as other states, payers and purchasers implement health care reform.
Each year, hospital-acquired infections, prescribing and treatment errors, lost documents and test reports, communication failures, and other problems have caused thousands of deaths in the United States, added millions of days to patients' hospital stays, and cost Americans tens of billions of dollars. Despite (and sometimes because of) new medical information technology and numerous well-intentioned initiatives to address these problems, threats to patient safety remain, and in some areas are on the rise.
In First, Do Less Harm, twelve health care professionals and researchers plus two former patients look at patient safety from a variety of perspectives, finding many of the proposed solutions to be inadequate or impractical. Several contributors to this book attribute the failure to confront patient safety concerns to the influence of the "market model" on medicine and emphasize the need for hospital-wide teamwork and greater involvement from frontline workers (from janitors and aides to nurses and physicians) in planning, implementing, and evaluating effective safety initiatives.
Several chapters in First, Do Less Harm focus on the critical role of interprofessional and occupational practice in patient safety. Rather than focusing on the usual suspects—physicians, safety champions, or high level management—these chapters expand the list of "stakeholders" and patient safety advocates to include nurses, patient care assistants, and other staff, as well as the health care unions that may represent them. During that day and half, Brian Sinclair, a middle-aged, non-Status Anishinaabeg resident of Manitoba's capital city, arrived in the emergency room of the Health Sciences Centre, Winnipeg's major downtown hospital, was left untreated and unattended to, and ultimately died from an easily treatable infection. His death reflects a particular structure of indifference born of and maintained by colonialism.McCallum and Perry present the ways in which Sinclair, once erased and ignored, came to represent diffuse, yet singular and largely dehumanized ideas about Indigenous people, modernity, and decline in cities. This story tells us about ordinary indigeneity in the City of Winnipeg through Sinclair's experience and restores the complex humanity denied him in his interactions with Canadian health and legal systems, both before and afterhis death.Structures of Indifference completes the story left untold by the inquiry into Sinclair's death, the 2014 report of which omitted any consideration of underlying factors, including racism and systemic discrimination.

As a consequence, it has entered the national consciousness as a key data point potentially supporting the block grant approach to Medicaid reform. In this book, Edward Alan Miller identifies factors that either facilitated or impeded the design and implementation of Rhode Island’s Global Consumer Choice Compact Medicaid Waiver in order to draw broader lessons for the Medicaid block grant debate and health and long-term care reform more generally. Evidence gathered from archival sources and in-depth interviews with key stakeholders exposes the role that provider capacity has played in the implementation process, including adult day care, assisted living, home maker, and other home- and community-based services. The impact of the Global Waiver on the nursing home sector is examined as well, in addition to new authority to obtain federal matching dollars for previously state-only funded programs. By providing a sophisticated understanding of factors enhancing or impeding state health reform, this book will contribute to improvements in the development and administration of policy development at both the state- and federal-levels.
