Ideal for physicians and residents from a wide range of medical and surgical disciplines, this unique title details new methods of imaging the sacral plexus, as well as its pathology and appropriate surgical approaches.

  • Demonstrates the surgical anatomy of each branch of the sacral plexus using fresh cadaveric dissections.
  • Color-codes nerves to differentiate them from other tissues and dissects them in a layer-by-layer manner.
  • Complies the knowledge and expertise of renowned clinical anatomists and researchers Dr. R. Shane Tubbs and Dr. Joe Iwanaga in this key area of surgical anatomy

Описание: The earliest human known to have the capability of walking erect on two legs is Sahelanphropus, who lived 6 million years ago. The ability to stand erect led to required extensive changes in the human skeleton, including significant changes in the cervical spine. In modern humans, the cervical spine holds the head upright and gives it great mobility. The combination of great mobility in this spinal segment combined with the requirement that it carry significant weight makes the cervical spine susceptible to a wide variety of pathologies. The cervical spine not only supports the head upright, but acts as a channel for the full set of neural elements connecting the brain with all near and distant parts of the body; thus, pathologies involving the spinal column in this segment directly affect the cervical spinal cord and exiting nerve roots. Today, excellence in spine surgery requires a thorough understanding of spinal anatomy, relevant neurology, and biomechanics, as well as skilled use of a variety of surgical techniques. The surgeon must master the ability to effectively select from a wide and constantly changing variety of alternative instrumentations and surgical approaches. In addition, the trend towards reducing the invasiveness of surgical procedures has led to the use of smaller and smaller tools and smaller surgical incisions with more limited views of the relevant anatomy. As a result of the rapid pace of change, the choice of an optimal technique in any given situation is increasingly complex. In this book, we begin with a basic review of anatomy, neurology, neurophysiology, and biomechanics. We also discuss clinical and radiological assessment required for a differential diagnosis, and present a thorough discussion of the importance of sagittal alignment of the spine and the utility of gait analysis. We proceed with a thorough discussion of nontraumatic pathologies causing cervical myelopathy, beginning with the craniocervical junction down through the subaxial spine, in pediatric and adult populations. This discussion includes steps in the differential diagnosis for specific pathologies, surgical techniques and nuances, radiation-based treatment alternatives, and special topics ranging from the use of stem cells to robotics and endoscopic surgery. We have attempted to provide both fundamental and state-of-the-art knowledge and to share the rich experience of some of the leading spine surgeons worldwide, with the aim of enabling surgeons at all levels to advance their own capabilities for performing safe and successful procedures in this area of complex anatomy.

Описание: The definitive textbook on the management of cervical spine trauma from master spine surgeons! Understanding the clinical implications of cervical trauma requires thorough knowledge of the anatomy and physiology of the cervical spine. Cervical Trauma: Surgical Management by renowned spine surgeon Robert Heary and a cadre of prominent neurosurgical and orthopaedic spine experts is the most comprehensive, state-of-the-art resource available to date on this topic. The text begins with discussion of cervical anatomy and the pathophysiology of spinal cord injury (SCI), SCI classification systems, initial assessments in patients with cervical SCIs, and cranioskeletal traction, followed by injury-specific chapters. Classification systems and management protocols developed over the last 40 years have enabled spine surgeons to work collaboratively with specialists in trauma surgery and critical care to provide optimal management of SCIs and attain improved long-term patient outcomes. This book covers a full spectrum of trauma-related conditions impacting the cervical spine and multidisciplinary interventions including minimally invasive surgery, neurointerventional techniques, reconstructive therapy with bone grafts or alternative stabilization methods, evidence-based medications, and SCI rehab. Key Highlights Discussion of upper cervical injuries – from more prevalent trauma such as atlanto-occipital injuries, odontoid and hangman's fractures, and atlantoaxial subluxations – to uncommon injuries like atlantoaxial rotatory fixation Management of subaxial injuries in adults and children and cervical burst fractures Special topics including sport-related cervical spine injuries and return-to-play criteria, craniovertebral injuries in pediatric patients, and managing comorbidities such as congenital spinal stenosis and rheumatoid arthritis Pearls on handling potential complications and insightful guidance and rationales for choosing surgical interventions over conservative methods and vice versa Neurosurgical and orthopaedic residents, veteran spine surgeons, and allied healthcare practitioners who treat patients with traumatic cervical spine conditions will benefit from reading this outstanding resource, cover-to-cover. It also provides an ideal go-to reference to consult in the ER when patients present with cervical trauma. This book includes complimentary access to a digital copy on https://medone.thieme.com.

Описание: Fifty-two patients operated on for this type of spondylogenic myelo- pathy by the anterior approach were reexamined. The male: female re- lation was 2: 1. Fifty percent were males in the 5th decade. About 20% had a congenital narrow spinal canal with an ap-diameter below the minimal standard values (3). The maximum of spondylogenic narrow- ing was found between C4 and C6 in 2/3 of the cases. Results: good 44%, satisfactory 31%, unchanged 13%, deteriorated 12%, mortality O. The duration of the neurological symptoms before operation had no in- fluence on the surgical results. Final neurological restitution was reached within 1 week to 3 months after the operation. The operative results in patients with congenital narrow spinal canal showed the same percentual distribution as the results of the entire group. No factors could be found to explain the groups of unsatisfactory results. References 1. DECKING, D., STEEGE, W. ter: Rontgenologische ParametAr der Hals- wirbelsaule im seitlichen Strahlengang.
