How can country authorities implement a new MTEF or strengthen an existing one? Beyond the Annual Budget provides a comprehensive review of global experience with Medium Term Expenditure Frameworks (MTEFs). Looking at countries both with and without MTEFs over the period 1990 to 2008, the authors adopt a systematic methodological approach and rely on multiple analytical techniques—including event studies and econometric analysis—to obtain results about the impact of MTEFs on fiscal performance. The authors then draw on case studies and other material to determine whether MTEFs should be a common element of public financial management systems given differences in country circumstances. Guidance is also provided on the design and implementation of MTEFs in the context of broader public financial management systems reform. This volume will be of interest to multilateral and bilateral providers of technical assistance in the public financial management area, and to country authorities seeking to introduce or strengthen MTEFs.|This study shows how power was constructed, enacted, and contested by discursive and non-discursive strategies and practices. This study examines and explains the relationship between social health insurance (SHI) participation and out-of-pocket expenditures (OOP) as well as the mediating role the institutional arrangement of SHI plays in this relationship in China. Embracing a new institutionalist approach, it develops two analytical perspectives: determination, which identifies the mechanisms of social health insurance, and strategic interaction, which explores the interaction among social health insurance agencies, healthcare providers, patients, and institutions. It reveals the poor performance of social health insurance in decreasing out-of-pocket health expenditures caused by a trade-off between the reimbursement, behavior management, and purchasing mechanisms of social health insurance programs. Further, it finds that the inequitable allocation of healthcare resources and patients’ concerns regarding the benefits offset the strategies used by social health insurance agencies to manage care-seeking behavior. It also discovers that the complex interactions between insurance agencies, doctors, patients and a larger disenabling institutional surrounding restricts the purchasing efficiency of social health insurance. This book is characterized by its unique synthesis of the role of the institutional arrangement of social health insurance in China, the interaction between the stakeholders in health sectors, and of the relationship between healthcare institutions, actors, and policy outcomes. Providing a comprehensive overview, it enables scholars and graduate students to understand the ongoing process of social health insurance reform as well as the dynamics of health cost inflation in China. Advanced nations are headed for a new era of bigger government, with government expenditure set to increase enormously over the next three decades. In all advanced economies, health expenditure will be driven irresistibly upward by the cost of precision medicine and other new technologies, as well as by the need to repair weaknesses highlighted by the coronavirus pandemic. Governments will also be compelled to spend big on fighting global warming, fixing ailing infrastructure and providing adequate aged care for a rapidly graying populations. The combined effect of these and other spending pressures threatens to drive budget deficits and government debt through the roof. Substantial tax increases will be necessary, most of all in the low-tax United States. Pruning welfare spending will also be high on the agenda in countries with the most comprehensive welfare states. Printing money - as proposed by advocates of modern monetary theory - is not a solution, but a sure route to disaster. Faced with this reality, the political right must abandon pipedreams of small government. The left, on the other hand, needs to recognize that there is no room or need for costly projects like a universal basic income. Mankind is not facing the threat of mass technological unemployment, and government spending needs to be focused on addressing real problems rather than imaginary ones.
