Описание: The Affordable Care Act’s impact on coverage, access to care, and systematic exclusion in our health care system The Affordable Care Act set off an unprecedented wave of health insurance enrollment as the most sweeping overhaul of the U.S. health insurance system since 1965. In the years since its enactment, some 20 million uninsured Americans gained access to coverage. And yet, the law remained unpopular and politically vulnerable. While the ACA extended social protections to some groups, its implementation was troubled and the act itself created new forms of exclusion. Access to affordable coverage options were highly segmented by state of residence, income, and citizenship status. Unequal Coverage documents the everyday experiences of individuals and families across the U.S. as they attempted to access coverage and care in the five years following the passage of the ACA.It argues that while the Affordable Care Act succeeded in expanding access to care, it did so unevenly, ultimately also generating inequality and stratification. The volume investigates the outcomes of the ACA in communities throughout the country and provides up-close, intimate portraits of individuals and groups trying to access and provide health care for both the newly insured and those who remain uncovered. The contributors use the ACA as a lens to examine more broadly how social welfare policies in a multiracial and multiethnic democracy purport to be inclusive while simultaneously embracing certain kinds of exclusions. Unequal Coverage concludes with an examination of the Affordable Care Act’s uncertain legacy under the new Presidential administration and considers what the future may hold for the American health care system. The book illustrates lessons learned and reveals how the law became a flashpoint for battles over inequality, fairness, and the role of government. More books on the health care debate

Описание: This paper presents unique evidence on health workers' career choices in Ethiopia. It shows that challenges like health workers' limited willingness to work in rural areas, as well as their likelihood to migrate abroad vary substantially and are correlated with background, motivation, and job satisfaction. Governments in Africa have identified human resources for health as a priority to improve health outcomes. This study is a valuable resource to better understand health worker choices and help toward the design of more effective human resource policies. This working paper was produced as part of the World Bank's Africa Region Health Systems for Outcomes (HSO) Program. The Program, funded by the World Bank, the Government of Norway, the Government of the United Kingdom and the Global Alliance for Vaccines and Immunization (GAVI), focuses on strengthening health systems in Africa to reach the poor and achieve tangible results related to Health, Nutrition and Population. The main pillars and focus of the program center on knowledge and capacity building related to Human Resources for Health, Health Financing, Pharmaceuticals, Governance and Service Delivery, and Infrastructure and ICT.

Ideas about health are reinforced by institutions and their corresponding practices, such as donning a patient's gown in a hospital or prostrating before a healing shrine. Even though we are socialized into regarding such ideologies as "natural" and unproblematic, we sometimes seek to bypass, circumvent, or even transcend the dominant ideologies of our cultures as they are manifested in the institutions of health care. The contributors to this volume describe such contestations and circumventions of health ideologies, and the blurring of therapeutic boundaries, on the basis of case studies from India, the South Asian Diaspora, and Europe, focusing on relations between body, mind, and spirit in a variety of situations. The result is not always the "live and let live" medical pluralism that is described in the literature.

Описание: This book presents the healthcare reform experiences of six small- to mid-sized, but dynamic, economies spanning the Asia-Pacific, the Middle East and Europe. Usually not given serious consideration in major international comparisons because of their small size, each in fact provides a fascinating case study that illuminates the understanding of the dynamics of healthcare reform. Although dissimilar in historical and cultural backgrounds, they share some important features: all faced very similar pressures for change in the 1970s and 1980s; all considered a very similar range of policy options; and all did not only discuss but actually implemented fundamental changes in their healthcare funding, organization, contracting and governance structures with strikingly different outcomes. All of the authors have lived and worked in one or more of the countries studied in this volume. The analytic frameworks they use reflect their broad range of professional and disciplinary backgrounds in health economics and political science. Beyond mere descriptions of reform processes and superficial analyses based on aggregate data from the usual OECD or WHO sources, they seek to understand -- and explain -- the variations in country experiences by examining the politico-socio-economic factors driving health reform as seen through the respective country lenses. In coming together in this unique international collaboration, they make an important contribution to the growing field of international comparative health policy studies.Contributors: Tsung-Mei Cheng (Princeton University, USA), David Chinitz (The Hebrew University of Jerusalem, Israel), Luca Crivelli and Iva Bolgiani (University of Lugano, Switzerland), Meng-Kin Lim (National University of Singapore, Singapore), Kieke G H Okma and Hans Maarse (Maastricht University, The Netherlands), Toni Ashton and Tim Tenbensel (University of Auckland, New Zealand).
